2018 Breakaway Basketball
It's Coming!
January 13 - February 24 (Saturdays Only)

Breakaway Basketball is a community event that is hosted by Perth Bible Church for boys and girls in grades 1-12. The cost is $30 per child until December 31st, 2017. On January 1st, 2018 the price will increase to $30 per child. Please feel free to contact us with any questions you may have! If you'd like to register your child today, you can do so by filling out the forms below.

The following instructions apply to the payment form.

1. Enter 25 as the Amount
2. Select Breakaway Basketball as the Fund
3. First Name and Last Name must match parent/guardian name listed on the waiver

I UNDERSTAND

*January 13 is a starter clinic!*

*1st-6th Grade: 8:30am-10:30am

*7th-12th Grade: 10:30am-12:30pm

All Other Weeks Are As Follows:

1st-3rd Grade: 8:30am-10am

4th-6th Grade: 10:15am-11:45am

7th-12 Grade GIRLS: 12pm-1:30pm

7th-12th Grade BOYS: 1:45pm-3:15pm
